Liberty is pleased to provide facility for ministry uses. Please complete the form and email to our Church Administrator: email@example.com. Completing the form does not guarantee space until you receive a confirmation email from the Church Administrator.
Ministry Leaders, please complete the following purchase request form BEFORE making purchase. Even if area has been budgeted it needs to be cleared and confirmed by the finance office. Approved purchase requests, may then be submitted for reimbursement using form if applicable. All forms must be turned in to Finance Secretary Martha Cain: firstname.lastname@example.org
